Background:

Compared to the frequency of occurrence of pre-auricular skin tag, cervical chondrocutaneous branchial remnants is one of congenital, benign neck masses that is very rare all over the world. Most of these rare anomalies are reported in case reports and especially, rare cases of unilateral cervical chondrocutaneous branchial remnants have been reported in Korean.

Materials & Methods:

A 9-year-old male patient visited the hospital on September 2017 for a rod-shaped mass. As a simultaneous diagnosis and treatment method, complete surgical excision was executed.

Results:

Excised mass was 0.5cm in diameter, 1.2cm in. Histologically, a hyaline and elastic cartilage was found in the core. As a family history, the same remnant was found in the right Anterior neck area of his mother.

Conclusion:

In this case, it is possible to diagnose and treat simply at the same time and even an optimal aesthetic result can be obtained.

서론

새성기관은 태생 2주에 발생하여 6주에서 7주 사이에 여러 구조물로 나타나게 되는데, 이러한 새성기관의 유물(remnants)로는 낭종(cyst), 루(fistula), 또는 연골(cartilage) 등으로 나타난다. 이중에서 경부 새성기형(Cervical branchial remnants) 는 귀앞에 주로 생기는 연성 섬유종(skin tag) 과 비교하여 그 빈도가 드물게 보고되고 있다. 주로 염증소견이 없고 통증이 없는 경우에는 무통성의 종물을 형성하여 연골조직이 내부에 존재하는 양상으로 관찰되게 되며 미용적인 목적이나 진단적인 목적으로 수술적 치료를 시행하게 된다. 특히 경부피부연골새성기형(cervical chondrocutaneous branchial remnant)의 경우에는 다른 기형의 동반 가능성이 있다는 보고가 있다.1,2)

이에 본교실에서는 전세계적으로 매우 드물게 발생하는 경부피부연골새성기형의 증례를 경험하고, 진단적 수술치료에 대하여 고찰과 함께 보고하는 바이다.

증례

2017년 9월에 9살 남자 환아가 우측 목 앞쪽부위에 돌출된 살덩어리를 주소로 내원하였다. 태어날 때부터 존재하였고, 환아의 성장과는 무관하게 크기가 유지는 양상을 보였으며, 진물이 나오거나 아프거나, 붉게 염증소견을 보인 적은 없었다고 하여 미용적인 목적과 정확한 진단과 치료를 위해 내원하였다. 신체 검진상에서는 0.5cm 정도의 길이의 원기둥 모양이었고 내부 중앙에 연골양상의 1cm 정도 길이의 촉진이 되는 부위가 관찰되었고 압통, 박동성은 없었다. 위치는 흉쇄유돌근의 목 앞쪽 경계 하방 3분의 1 지점정도로 관찰되었다(Fig. 1). 우선 목 내부와 연결되는 루나 낭종, 이상혈관의 가능성을 배제하기 위해서 초음파를 시행하였으며, 초음파 영상학적 소견으로는 맹관으로 관찰되었고, 주변에 이상혈류나 낭이 관찰되지 않았으며 저음영(hypoechoic) 소견을 보였다(Fig. 2). 치료와 진단을 하기위한 수술적인 방법을 고려하였으며 수술전 시행한 단층촬영술에서도 목 내부로 연결되는 부위는 관찰되지 않았고 피부와 그 안쪽에 저음영의 소견을 보이는 원기둥모양이 관찰되었다(Fig. 3).

치료와 진단을 하기위한 전신마취하 절제적 생검술을 시행하였으며, 절제된 종괴는 지름 0.5cm, 길이 1.2cm 정도의 원기둥모양의 중심부에 연골조직으로 구성된 것으로 관찰되었다(Fig. 4). 외부에서 보이는 피부조직 안쪽으로 연골로 의심되는 중심부 기둥이 형성되어 있었으며, 절제부위에 이상혈관이나 신경 등은 관찰되지 않았고 활경근(platysma)까지 침범하진 않아 근육손상이나 근육 교정술은 필요하지 않았다. 병리조직학적 소견으로는 피부, 진피, 피하지방, 유리연골을 포함하는 경부 피부연골새성기형으로 진단되었다(Fig. 5). 이외에 동반 기형은 관찰되지 않았다.

고찰

경부 피부연골 새성기형은 드물게 발생하는 선천성, 양성 이소종(choristoma)으로 볼 수 있다.3) 대부분의 목 부위에 나타나는 선천기형은 새성기관의 구조물들이 전환(transformation) 하는 동안 발생하게 된다.4)

1958년 증례 보고를 시작으로 1997년에는 Atlan 에 의해 가장 큰 경부 피부연골 새성기형이 보고되었다.5,6,7) 이후 2017년까지 양측성 34 증례를 포함한 총111개의 증례만 보고될 정도로 드문 기형이다. 크게 분류하면 발생 원인으로는 여분 이개부위 조직(ectopic auricular tissue)이라는 이론과 또다른 이론은 경부조직(branchial tissue)에서 기원하였다는 것이다. 이중에서 탄력연골(elastic cartilage)의 존재하는 경우는 제 1,2 새궁(first or second branchial arch)에서 기원되는 이개부위와 제 4~6 새궁(4th~6th branchial arch)에서 기원되는 경부 하방에서도 발생할 수가 있지만, 유리연골(hyaline cartilage)이 존재한다면 이 소견은 이개부위 기원이 아닌 경부 기원인 것으로 판단할 수 있다. 이 기형은 모두 단측성과 양측성으로 발생이 가능하며, 일반적으로 선천 경부 피부연골새성기형은 남성에서 호발하고, 흉쇄유돌근의 하방 3분의 1지점 경부에 발생하는 임상적 양상을 보이고 있다.8) 피부로 감싸 있는 돌기모양으로 염증이나 감염소견, 압통이나 통증은 보이지 않는 특징을 가지고 있고, 목내부로 연결되는 루나 조직은 없이 수술적인 치료로 수월하게 치료가 가능한 부분이다.9)

귀 앞 부위에 주로 존재하는 피부섬유종(skin tag)은 부속 이주(accessory tragus)의 동반 가능성도 많고 빈도가 높은 반면, 경부 피부연골새성기형에 대한 보고는 전세계적으로 드물게 보고되고 있으며 특히 가족력을 보이는 보고는 더욱 드물다.10) 이런 기형의 경우 동반되는 선천기형의 가능성이 보고된 바 있어서 이비인후과적인 부분, 비뇨기과적인 부분, 심장학적인 부분, 근골격계부분 등의 문제에 대한 전체적인 확인이 필요한 기형이다.11) 또한 경부 새성기형과 귀앞 피부 섬유종은 Goldenhar, Treache-Collins, Nagar 등의 증후군과 동반된 보고도 있어 추가적인 확인이 필요하다.12,13) 또한, 본 증례에서는 맹관으로 낭종이나 루 등의 동반된 부분이 없었지만 이런 것이 동반된 경우에는 동반 자체만으로도 수술적인 치료가 필요하며 4분의 1이상에서 염증이나 감염 가능성이 있어 이환률을 증가시키기 때문이라고 보고되고 있다.14)

본교실에서는 환아와 모친에 있어서 유사부위인 우측 흉쇄유돌근 하방 3분의 1부위에 진단된 단측성 경부 피부연골새성기형을 수술적인 치료와 진단을 시행하였으며, 유리연골의 존재를 확인하면서 목부위에 발생한 귀연골조직의 흔적이라기 보다는 경부조직 기원의 새성기형으로 판단하였다.

따라서 경부 피부연골새성기형이 의심되는 경우 전체적인 다른 동반 기형의 유무를 파악해야 하며, 미용적인 목적이나 치료적인 목적으로 수술적 치료를 고려한다면 정확한 신체 검진과 병력파악, 술전 영상학적 검사를 통한 수술부위의 확인이 필요하고, 수술적인 접근은 별다른 위험성 없이 수월하게 제거할 수 있는 방법으로 사료되어, 본 교실에서는 전세계적으로 드문 단측성 경부 피부연골새성기형의 증례를 보고하는 바이다.
